Registration Forms
To download the registration form please click on the link below. We would like to ask that teams interested in playing contact us with registration by March 17th, for the $10 discount. Registrations received after will cost the full price of $60 for an adult team and $50 for a youth team.
You can e-mail the form to hoop4hopeomaha@gmail.com or mail it to the address on the form. We look forward to seeing everyone there!
